Responsibilities
- Collect relevant data pertinent to the patient’s health or situation.
- Analyze assessment data to determine diagnosis and care issues.
- Develop a plan with prescribed interventions to attain outcomes; implement the plan and coordinate care delivery.
- Promote a safe environment and evaluate progress toward anticipated outcomes.
- Collaborate with the patient, family, and healthcare team; provide learning opportunities for patients, families, and team members; directly provide health information.
- Participate in discharge planning to ensure continuity of care; delegate appropriately and coordinate duties of healthcare team members; perform other job-related duties as assigned.

Requirements
- Active RN license (California) or current eligibility for California licensure.
- Graduation from an accredited nursing program (ADN or BSN).
- Ability to work night shifts (3x12) and participate in a travel assignment.
- Ability to provide patient- and family-centered care in a home healthcare/hospice setting.
- Strong communication and collaboration skills with interdisciplinary teams.
- Competence in essential nursing duties, patient education, and documentation in a home health/hospice environment.
