The Registered Nurse is responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. The role involves conducting comprehensive patient assessments, administering medications, and developing treatment plans while maintaining strong communication and collaboration with patients, providers, and the interdisciplinary care team.

Responsibilities:
- Provide direct skilled nursing care, case management, and hospice care to patients.
- Perform daily clinical assessments and monitor patient conditions.
- Administer medications safely and manage individualized treatment plans.
- Collaborate effectively with providers, patients, and interdisciplinary team members.
- Complete clinical documentation and administrative duties related to case management.
- Travel to various patient locations to deliver field-based care.
Required Skills & Certifications:
- Active Registered Nurse license in the state of practice.
- Minimum of two years of experience in hospital, home health, or hospice care.
- Bachelor's degree in Nursing or a related field.
- Valid driver's license, active car insurance, and a personal automobile for field work.
- Demonstrated proficiency in clinical assessment, medication administration, and treatment planning.
- Excellent communication, interpersonal, and interdisciplinary collaboration skills.
Preferred Skills & Certifications:
- Clinical specialty experience in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room environments.

- The position requires field work and reliable personal transportation to reach patients.
- The care environment focuses heavily on hospice care and assisting individuals facing advancing age or serious illness.
- Must be adaptable and capable of functioning effectively in a fast-paced environment.
Scheduling:
- 40 hours per week.
- Rotating shifts.
- Must be available to work consecutive weekends as needed based on scheduling demands.,
