Job Description

The Medical Transport Specialist reports directly to the Registered Nurse, then to the HCS Program Manager. This person primarily provides health and medical support to those receiving services through Community Homes for Adults, Inc.

The Medical Transport Specialist’s job performance will be rated on the following outcomes and performance measures:

  • Deliver routine medications, either prescription or non-prescription, to group homes
  • Provide transportation to and from medical and dental appointments in a manner that respects the appointment time and requirements
  • Notify appropriate parties of appointments being scheduled including day, time, location and purpose of visit and send appointment reminders prior to the scheduled appointment
  • Submit medical appointment information (ie after-visit summary, consultation forms, nursing notes, standing orders, etc) to the guardian and document in Therap following the appointment.
  • Respond to any physician office call backs for any medical needs.
  • Notify appropriate parties of any changes of client appointment schedule or medical information
  • Assisting with ensuring prescriptions are filled, refilled and/or called in
  • Make sure all appointments are added to the calendar in Therap and that appointments for the upcoming week are sent out in the form of appointment reminders
  • Maintain records with accurate and detailed information by updating the respective electronic health record, filing orders and medical forms, etc.
  • Assures that people are respected and that they are free from abuses of any kind. Staff will follow proper procedures when any abuses are noted or suspected.

In addition to the above, the Medical Transport Specialist is expected to: Demonstrate competence in driving the agency vehicle and show insurability. Provide a valid Texas Driver’s License. Where applicable, maintain current insurance on own

  • Must demonstrate competence in all areas of required training for position such as CPR, first aid, SAMA, Self-Administration of Medication, documentation, behavioral principles, and other trainings as applicable.
  • Must pass a drug test and be free of infectious and communicable disease. Must be able to bend, lift, stoop and assist all residents in their care and daily living activities as a requirement of the job.
  • Must meet and maintain a criminal history void of offenses listed on the DADS list of non-allowable offenses, offenses that represent a pattern of negligent fiduciary responsibility, or any behavior represented on the Employee Misconduct Registry

Qualifications: Employees must have and present a high school diploma or equivalency. They must have at least 1 year of work experience with the IDD community plus references to substantiate this time. The ideal candidate will have previous experience DSP/QIDP in the field of IDD. Medical degree and/or certification is not required. They must be 18 years old, meet and maintain the criteria listed above.

Language Ability: The Medical Transport Specialist must be able to read and interpret documents such as evaluations from referring agencies, schools, psychological evaluations, progress notes from direct care staff, and clinicians, after-visit summaries, etc. They must be able to write reports and provide documentation to be sent to outside agencies. They must be able to present verbally to the Interdisciplinary Team and be able to converse and interpret information to the individual in a manner consistent with his or her level of understanding.

Computer Skills: To perform this job successfully, the Medical Transport Specialist will require the ability to use the computer in word processing, the internet for research, email and software as it relates to the needs of the individuals.
